Sagility is seeking an experienced Senior Care Specialist (Registered Nurse) to work from home. The role involves telephonic health assessments, developing care plans, coaching members, and coordinating care across teams to support complex health needs.

Ideal candidates hold an active RN license, a compact license, and have 2+ years of clinical nursing experience along with strong communication skills. Seasonal, with an end date planned for June 2027.

Formación

• Active RN license in the state of residence.
• Compact RN license permitting practice in applicable states.

Responsabilidades

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized care plans based on member needs and risk.
• Establish health coaching relationships and provide education and support for chronic condition management.
• Use nursing judgment to assess needs and coordinate interventions for acute situations.
• Support transitions of care and discharge planning, including DME and home health services.
• Help members navigate benefits and care options.
• Assess factors affecting health and well-being.
• Collaborate with multidisciplinary teams.
