Job summary

Sagility is hiring a Senior Care Specialist (Registered Nurse) to perform telephonic health assessments and develop individualized care plans from home. This role focuses on Medicare, Medicaid, or group health plan members, coordinating care and coaching patients remotely.

The position requires an active RN license and a Compact license, with 2+ years of clinical nursing experience, strong communication skills, and comfortable working in a remote environment.

Qualifications

• Active, unrestricted Registered Nurse (RN) license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3 or more years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently and exercise appropriate nursing judgment in a remote environment.
• Intermediate to advanced computer proficiency, including Microsoft Word, Excel, and Outlook.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized, and measurable care plans based on member needs and identified health risks.
• Establish health coaching relationships with members and provide education, encouragement, and support related to chronic condition management.
• Use clinical nursing judgment to assess member needs and coordinate appropriate interventions for acute situations.
• Support transitions of care and safe discharge planning, including coordination of DME, home health services, provider appointments, and other resources.
