Sagility is seeking a Senior Care Specialist (Registered Nurse) to work from home. The role focuses on telephonic health assessments, personalized care plans, and coaching for Medicare/Medicaid populations.

Ideal candidates will hold an active RN license (state of residence) and a Compact RN license, with 2+ years of nursing experience. This seasonal, remote opportunity includes flexible pay options and access to earned wages prior to payday.

Competenze

• Active, unrestricted Registered Nurse (RN) license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Ability to work independently and in a remote environment.
• Intermediate to advanced computer proficiency including Word, Excel, and Outlook.
• Ability to navigate multiple EMR and computer systems.

Mansioni

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized, and measurable care plans.
• Establish health coaching relationships and provide education and support for chronic condition management.
• Use nursing judgment to assess needs and coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning, including DME and home health services.
• Help members navigate healthcare benefits and care options.
• Assess factors affecting health and well-being.
• Collaborate with multidisciplinary teams.
• Promote member self-management with education and resources.
• Coordinate care and management activities among members, caregivers, and providers.
• Document interactions, assessments, care plans, and outcomes accurately in systems.
