Job summary

Sagility seeks an experienced Senior Care Specialist (Registered Nurse) to perform telephonic health assessments from a work-from-home setting across US member populations. You will develop individualized care plans, coordinate interventions, and support transitions of care with a multidisciplinary team.

The role requires an active RN license, a Compact RN license, and strong communication skills. This seasonal opportunity accepts candidates nationwide and offers flexible remote work options.

Qualifications

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable states.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, or group health plan members.
• Develop actionable, individualized care plans based on member needs and health risks.
• Establish health coaching relationships with members and provide education and support for chronic condition management.
• Use clinical nursing judgment to assess member needs and coordinate interventions.
• Support transitions of care and safe discharge planning, including DME and home health coordination.
• Help members navigate healthcare benefits, services, and care options.
• Assess factors affecting health and well-being.
• Collaborate with multidisciplinary teams.
• Promote self-management with education and resources.
• Coordinate care among members, caregivers, and providers.
• Document interactions, assessments, and outcomes accurately in systems.
