Sagility is seeking a Senior Care Specialist (Registered Nurse) to work from home in a seasonal capacity. You will conduct telephonic health assessments, develop individualized care plans, and provide health coaching to members across Medicare/Medicaid and group health plans.

This remote RN position requires independent work, strong clinical judgment, and fluency with EMR systems while collaborating with a multidisciplinary team to coordinate care and outcomes.

Competenze

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment and problem-solving skills.
• Ability to work independently in a remote environment.
• Intermediate to advanced computer proficiency including Word, Excel, and Outlook.
• Ability to navigate multiple computer systems and electronic medical records.
• Strong verbal and written communication skills.

Mansioni

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized, and measurable care plans based on member needs and health risks.
• Establish health coaching relationships with members and provide education and support for chronic condition management.
• Use nursing judgment to assess needs and coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning, including DME and home health services.
• Help members navigate healthcare benefits, services, and care options.
• Assess factors affecting health and well-being and coordinate care activities.
• Collaborate with multidisciplinary teams.
• Promote self-management with education and resources.
• Document interactions, assessments, and outcomes accurately in systems.
