Job summary

Call 4 Health is seeking an experienced Registered Nurse to join our 24-hour remote triage team. In this role you will perform phone-based triage, document in client EHR systems, and provide clinical assessments to patients across multiple states from a home office.

A compact nursing license, 2–4 years of RN experience, and strong communication are required. Flexible scheduling and nationwide coverage are offered.

Qualifications

• Associate’s or Bachelor’s Degree in Nursing.
• 2–4 years of full-time RN clinical experience, preferably in ER/Urgent Care, Adult, Pediatric, Ambulatory Care, or ICU.
• Active, unencumbered ENLC license and willingness to obtain additional state licenses.

Responsibilities

• Respond to inbound and outbound calls for a multi-state nurse triage service.
• Utilize established triage protocols to assess and triage patients.
• Document patient interactions accurately in client EHR systems.
• Provide clinical assessments via phone or patient portal.
• Support providers with administrative tasks including referrals and prior authorizations.
• Coordinate with hospitals, nursing homes, and other agencies as needed.
• Participate in Remote Patient Monitoring programs, including vital signs monitoring.
