Job summary

Call 4 Health is seeking an experienced Registered Nurse (RN) for remote triage work supporting providers nationwide. This role involves telephone triage, EHR management, and clinical support across multiple states from a home-based setup.

Ideal candidates have 2–4 years of RN experience, current nursing licensure, and familiarity with multiple EHR systems. The position offers flexible, full-time hours with remote collaboration and ongoing clinical duties.

Qualifications

• Associate's or Bachelor's Degree in Nursing.
• 2-4 years of RN clinical experience.
• Prior telephone triage experience using electronic triage systems.
• Experience with two or more EHR platforms.
• Active, unencumbered e-NCL/ Enhanced Nurse Licensure Compact license.
• Willingness to obtain additional state licenses.

Responsibilities

• Respond to inbound and outbound calls for a multi-state nurse triage service.
• Utilize Schmitt-Thompson triage protocols to assess patients.
• Document patient interactions accurately in client EHR systems and Call 4 Health platform.
• Provide clinical assessments via phone or patient portal per established protocols.
• Support providers with administrative and clinical tasks, including medication refills and post-discharge follow-up.
• Facilitate referrals and assist with prior authorization requests requiring clinical input.
• Collaborate with hospitals, nursing homes, government agencies as needed.
• Participate in Remote Patient Monitoring programs: monitor vital signs and educate on disease management.
• Screen and qualify patients for clinical trials and provide patient education.
• Take on additional responsibilities to support team success and patient outcomes.
