Call 4 Health is hiring an experienced Registered Nurse (RN) for remote triage support across multiple states. You will perform telephone triage, document in EHRs, and provide clinical assessments via phone or patient portal from a home-based setup.

Ideal candidates have 2–4 years of RN experience, active nursing license, and familiarity with triage protocols and multiple EHR platforms. This remote role offers flexible scheduling while maintaining high-quality patient care.

Qualifications

• RN license (active and in good standing)
• 2–4 years of full-time RN clinical experience
• Experience with telephone triage
• Experience using two or more EHR platforms
• Willingness to obtain additional state licenses as requested

Responsabilités

• Triage inbound and outbound calls for a multi-state service
• Follow established telephone triage protocols
• Document patient interactions accurately in client EHR systems
• Provide clinical assessments via phone or patient portal
• Support providers with administrative and clinical tasks (medication refills, critical results, post-discharge follow-up)
• Facilitate referrals and prior authorization support
• Collaborate with hospitals, nursing homes, and other agencies
• Participate in Remote Patient Monitoring programs (vital signs, chronic disease education)
• Screen and qualify patients for clinical trials
• Take on additional duties to support team and patient outcomes
