Sentara Health in Roanoke, VA is seeking an Integrated Case Manager, RN to coordinate care across the continuum. The role requires travel to Roanoke and northern areas, with in-person assessments up to 50-75% of the week, and telephonic clinical assessments.

The Integrated Case Manager develops and revises care plans, identifies high-risk patients, and works with medical directors and the care team to optimize outcomes while ensuring regulatory and policy compliance.

Qualifications

• Registered Nurse with current RN licensure.
• Minimum 3 years of nursing experience.
• Managed care experience preferred.
• Discharge planning experience preferred.

Responsibilities

• Develop, monitor, and revise member care plans to optimize health care across the continuum.
• Perform telephonic clinical assessments for the identification, evaluation, coordination and management of member needs.
• Coordinate high-risk member care with the health care team and providers.
• Ensure compliance with regulatory, accrediting and company policies.
