Job summary

Sentara Health is seeking an Integrated Case Manager, RN to perform telephonic clinical assessments and coordinate care across physical and behavioral health, social services, and long-term care. The role develops and revises care plans to meet member needs and optimize health outcomes within the care continuum.

The position requires travel up to 60–90 minutes to see members, with an expected 50–75% in-person engagement weekly. Comprehensive benefits are provided.

Qualifications

• RN licensure preferred and relevant nursing experience.
• Bachelor's degree preferred.
• Managed care and discharge planning experience preferred.

Responsibilities

• Perform telephonic clinical assessments to identify, evaluate, coordinate and manage member needs.
• Develop, monitor and revise the member's care plan to optimize health across the care continuum.
• Identify high-risk members and coordinate care with the health care team.
• Manage chronic illnesses, co-morbidities and disabilities to ensure cost-effective care.
• Facilitate authorizations/referrals within benefits structures or extra-contractual arrangements.
• Interface with Medical Directors and care teams to review treatment plans and ensure compliance.
