Job summary

Sentara Health is hiring an Integrated Case Manager, RN. This remote role requires in-person assessments and travel up to 60-90 minutes to see members 50-75% of a week.

The candidate will develop and monitor care plans, perform telephonic assessments, and coordinate care across physical and behavioral health, social services, and long-term care needs. The role involves high-risk identification, collaboration with care teams, and ensuring compliance with policies.

Qualifications

• Bachelor’s degree in a related field preferred.
• Experience in managed care and discharge planning preferred.
• Ability to perform telephonic clinical assessments and care coordination.

Responsibilities

• Develop, monitor, evaluate and revise the member’s care plan across the care continuum.
• Perform telephonic clinical assessments for identification, evaluation and management of member needs.
• Identify high-risk members and coordinate care with the health care team.
• Interface with Medical Directors and Inter-Disciplinary Teams on care plans and ensure regulatory compliance.
• Travel to meet members and support in-person assessments as required.
