Job summary

Sentara Health is seeking an Integrated Case Manager, RN, to deliver case management services for members across physical, behavioral health and social supports. The role requires travel for in-person assessments up to 60-90 minutes to see members, with travel up to 50-75% of the week.

The incumbent will develop and monitor individualized care plans, perform telephonic assessments, and coordinate authorizations/referrals within benefit structures.

Qualifications

• RN license required or active RN in good standing.
• Experience in managed care and discharge planning preferred.
• Knowledge of coordinating medical and behavioral health services.

Responsibilities

• Perform telephonic and in-person assessments to identify member needs.
• Develop, monitor and revise member care plans across care continuum.
• Coordinate authorizations/referrals within benefits structure and external programs.
• Collaborate with Medical Directors and interdisciplinary teams to implement care plans.
• Support regulatory, accreditation, and policy compliance.
