Job summary

Sentara Health is seeking an Integrated Case Manager, RN. This remote role requires in-person assessments and may involve travel up to 60-90 minutes to support members weekly.

You will manage and coordinate member care plans across physical and behavioral health, social services, and long-term care, aiming to optimize health outcomes. Applicants should have case management experience and a Bachelor’s degree preferred; managed care and discharge planning experience are advantageous.

Qualifications

• Strong communication and collaboration with care teams.

Responsibilities

• Conduct telephonic and in-person assessments to evaluate member needs.
• Develop, monitor, and revise care plans across physical, behavioral health, social services, and long-term care.
• Coordinate authorizations/referrals within benefits oralternate arrangements as permissible.
• Interface with Medical Directors and care teams to ensure compliant, cost-effective care.
• Identify high-risk members and coordinate care across the continuum.
