Job summary

Enlyte is seeking a Field Case Manager (RN) to coordinate and implement patient treatment plans. This full-time, remote role requires local travel for in-person appointments and collaboration with physicians, employers, and patients to achieve optimal recovery and work readiness.

You will assess injuries, propose plans, and adjust goals with care teams, while handling workload independently and ensuring cost-effective care delivery. Excellent communication is essential.

Qualifications

• Education: Associates or Bachelor's in Nursing.
• Experience: 2+ years clinical practice; workers’ compensation exp preferred.
• RN license active and in good standing; URAC-recognized case management certs preferred.
• Proficiency with Excel, Word, and Outlook; strong written and verbal communication.
• Willingness to travel locally for in-person appointments; reliable transportation.

Responsibilities

• Apply case management standards to develop and implement treatment plans.
• Advise on injury types, medications, and job duties to facilitate recovery.
• Interview injured workers to assess recovery status and occupational factors.
• Collaborate with physicians and employers to tailor treatment plans.
• Modify job duties with employers to support early return to work.
• Manage workload, prioritize cases, and prepare status updates for customers.
• Attend appointments as needed and perform other duties as assigned.
