Job summary

Enlyte is seeking a Telephonic Case Manager to support injured workers and patients across the United States. You will develop and implement treatment plans with physicians, employers, and patients to ensure safe, cost-effective care and early return to work.

The role requires an active RN license, URAC case management certification, and 2+ years of clinical practice, with strong communication and independent work skills.

Qualifications

• Active RN license in good standing.
• URAC-recognized certification in case management (CCM, CDMS, CRC, CRRN or COHN, COHN-S, RN-BC, ACM, CMAC, CMC).
• 2+ years clinical practice preferred.
• Reliable internet access.
• Proficiency with Excel, Word, Outlook.

Responsibilities

• Demonstrate knowledge, skills, and competency in the application of case management standards of practice.
• Use advanced knowledge of injury types, medications, comorbidities, treatment options, and job duties to advise on a treatment plan.
• Interview disabled persons to assess overall recovery and occupational involvement.
• Collaborate with treating physicians/providers to tailor treatment plans.
• Work with employers and physicians to modify job duties for early return to work.
• Evaluate and modify case goals based on patient progress and treatment effectiveness.
• Independently manage workload, prioritizing cases and workflows.
• Prepare status updates for customers and assist in training new staff.
