Job summary

Option Care Health is seeking a part-time Clinical Transition Educator (RN) to educate patients, families, and facility staff on home infusion care pathways. You will collaborate with the sales team, perform patient assessments, and coordinate hospital-to-home transitions, using our platforms to document education and protect confidentiality.

The role requires an active RN license, 2+ years in healthcare, strong relationship and presentation skills, and willingness to travel to care facilities

Qualifications

• Active and unrestricted RN license required.
• Minimum of 2 years of healthcare industry experience.
• Experience building relationships with individuals at all levels of organizations.
• Knowledge of ACHC/URAC standards.
• Experience providing customer service to internal and external customers.
• Proficient with MS Office and internet/email.
• Willingness to travel to care facilities, hospitals, and clinics.

Responsibilities

• Educate patients, caregivers and facility staff about OCH services for successful transitions.
• Conduct patient assessments to determine eligibility for the care model.
• Collaborate with sales to align clinical education with market needs.
• Document education using OCH applications and protect patient confidentiality.
• Coordinate hospital-to-home transitions with facilities and care teams.
• Provide in-person or remote education and gather insurance information.
