Job summary

Sentara Health is seeking an Integrated Case Manager, RN to coordinate care across physical and behavioral health, social services, and long-term supports. This role involves remote work with in-person assessments and travel up to 60-90 minutes to see members about 50–75% of the week.

You will develop and monitor care plans, perform telephonic assessments, and collaborate with Medical Directors and interdisciplinary teams to optimize outcomes while ensuring regulatory compliance and efficient

Qualifications

• RN licensure required
• 3 years nursing experience required
• Managed care experience preferred
• Discharge planning experience preferred

Responsibilities

• Coordinate and manage member care plans across physical/behavioral health, social services and long-term supports.
• Perform telephonic clinical assessments to identify, evaluate, coordinate and manage member needs.
• Interface with Medical Directors, Physician Advisors and Interdisciplinary Teams on care plans.
• Present cases at case conferences for multidisciplinary input and ensure regulatory compliance.
