Sentara Health is seeking an Integrated Case Manager, RN to oversee case management across physical and behavioral health, social services and long-term supports. Qualifikationen

• Registered Nurse licensure required.
• 3 years of nursing experience required.
• Discharge planning experience preferred.

Aufgaben

• Manage case load and develop care plans within licensure scope.
• Perform telephonic clinical assessments to identify and coordinate member needs.
• Coordinate with medical directors, physicians and care teams on treatment plans.
• Identify high-risk members and facilitate authorizations/referrals within benefits.
• Ensure compliance with regulatory and company policies and procedures.
