Sentara Health is seeking an Integrated Case Manager, RN. This remote position requires in-person face-to-face assessments with travel up to 60-90 minutes to see members, roughly 50-75% of the week.

The role focuses on developing and monitoring care plans, coordinating with the health care team, and ensuring regulatory compliance while supporting patients across physical and behavioral health, social services and long-term care needs.

Qualifications

• RN certification is required.
• Associate degree required; Bachelor’s degree preferred.
• Minimum 3 years of nursing experience.
• Managed care experience preferred.
• Discharge planning experience preferred.

Responsabilités

• Perform telephonic and in-person case management assessments.
• Develop and revise member care plans to optimize health care.
• Coordinate with medical directors and care teams.
• Identify high-risk members and coordinate care with providers.
• Ensure compliance with regulatory and company policies.
