The role emphasizes advocacy, education, and adherence to regulatory guidelines. Ideal candidates hold an active Michigan RN license, with 3+ years clinical and 2+ years in case management; bilingual Spanish is a plus.

Qualifications

• Active and unrestricted MI RN license.
• 3+ years of clinical experience (diabetes, CHF, CKD, post‑acute, hospice, palliative, cardiac, Home Health) with Medicare.
• 2+ years in case management, discharge planning or home health coordination.
• Proficient with MS Office and healthcare software.
• Bilingual in Spanish.

Responsibilities

• Advocate for patients, enable effective communication and monitor progress.
• Coordinate case management across providers, insurers and families.
• Develop personalized care plans and facilitate services across continuum of care.
• Interact with members telephonically or in person; document activities.
• Collaborate with multidisciplinary team to maximize health outcomes.
• Ensure compliance with HIPAA and company policies.
