Job summary

Candidates must reside in Florida or Ohio, hold an active RN license, and bring 2+ years in case management or home health, with strong digital literacy and time-management skills.

Qualifications

• Active and unrestricted Compact RN licensure in FL or OH.
• 3+ years of clinical experience with Medicare members.
• 2+ years of case management, discharge planning or home health coordination.
• Proficiency with Word, Excel, Outlook and PowerPoint, plus basic proprietary apps.
• Able to work in a fast-paced, high-volume environment with strong time management and prioritization.

Responsibilities

• Coordinate and facilitate comprehensive care for individuals by assessing needs and developing personalized plans.
• Act as advocate for patients with effective communication and resource utilization.
• Prepare and file required documentation of case work activities.
• Interact with internal multidisciplinary team to maximize health outcomes.
• Communicate with providers, employers and insurers as appropriate.
• Adapt care plans to optimize wellness and outcomes and ensure timely return to baseline.
