CVS Health in Florida is seeking a Case Manager to coordinate comprehensive care for members, develop personalized care plans and collaborate with providers, insurers and community resources. The role focuses on advocacy, effective communication, and ensuring timely, quality outcomes for patients across her/his care continuum.

The position requires an RN license in FL or OH, 3+ years of clinical experience, and 2+ years in case management or home health coordination.

Qualifications

3+ years of clinical practical experience (diabetes, CHF, CKD, post-acute, hospice, palliative care, cardiac, Home Health) with Medicare members
2+ years of case management, discharge planning and/or home health care coordination experience
Proficiency with standard corporate software applications including Word, Excel, Outlook and PowerPoint
Able to work in a fast paced high volume environment with strong time management and prioritization skills
Efficient and effective computer skills including navigating multiple systems and keyboarding
Commitment to HIPAA guidelines and ethical handling of sensitive information

Responsibilities

Coordinate and facilitate comprehensive care for members by assessing needs and developing care plans
Advocate for patients and ensure effective communication among providers, members and families
Collaborate with internal multidisciplinary teams, providers, insurers and care coordinators
Assess medical and vocational status and develop plans to optimize wellness and outcomes
Document case work activities accurately and timely
Coordinate services across home health, discharge planning and community resources
