Job summary

Qualifications

• Active and unrestricted RN license in Michigan.
• Residence within listed Michigan counties as specified in job posting.

Responsibilities

• Conduct comprehensive assessments and determine care plans with member's care team.
• Develop and monitor plans of care to meet member goals.
• Collaborate with Medical Directors and programs to overcome barriers.
• Provide crisis intervention and follow-up as clinically indicated.
• Document care activities using a clinical case management platform.
