Job summary

CVS Health is seeking a Community Care Case Manager to coordinate comprehensive health services for individuals and families. You will assess needs, plan care, facilitate services, and advocate for options to promote quality, cost-effective outcomes across the care continuum.

You will liaise with members, families, employers, and providers, and collaborate with an interdisciplinary team to optimize wellness. Experience in Medicare, chronic conditions, and home health care is preferred.

Qualifications

• Active and unrestricted Compact RN licensure in OH or state of residence
• 3+ years clinical experience in areas like diabetes, CHF, CKD, post-acute care, hospice, palliative care or cardiac with Medicare members
• 2+ years in case management, discharge planning or home health care coordination

Responsibilities

• Liaise with members, families, employers, providers, and insurers
• Coordinate case management activities across the continuum of care
• Interact with members telephonically or in person, including home visits as needed
• Assess medical and vocational status; develop care plans to optimize wellness and outcomes
• Prepare documentation of case work activities and communicate with stakeholders
• Consult with multidisciplinary teams to maximize health outcomes
• Provide education and preventative information
• Comply with laws and regulations governing rehabilitation services
• Evaluate member needs and benefit plan eligibility using clinical tools
