Job summary

CVS Health seeks a Community Care Case Manager to coordinate assessment, planning, facilitation and advocacy for members and families across the care continuum.

The role requires active RN licensure, 3+ years clinical experience, and 2+ years in case management or discharge planning; responsibilities include home visits, telephonic outreach, and collaboration with physicians, employers, and insurers to optimize wellness and return to baseline.

Qualifications

• Active and unrestricted Compact RN licensure in OH or state of residence
• 3+ years clinical experience; preference for diabetes, CHF, CKD, post-acute care, hospice, palliative care with Medicare members
• 2+ years case management, discharge planning, and/or home health coordination

Responsibilities

• Coordinate assessment, planning, facilitation, care coordination, evaluation and advocacy for members and families.
• Act as liaison with providers, employers, insurers, and healthcare professionals to develop care plans.
• Perform home visits when needed, document all case work, and monitor progress toward optimal wellness and return to baseline.
