Qualifications

• RN license active and unrestricted in Michigan.
• 3+ years of clinical practice; Medicare experience preferred.
• 2+ years in case management, discharge planning, or home health coordination.
• Proficiency with Word, Excel, Outlook, PowerPoint and EHR systems.
• Able to work in a fast-paced, high-volume environment with strong time management.
• Excellent communication and problem-solving skills.

Responsibilities

• Coordinate and facilitate comprehensive care across healthcare providers.
• Assess patient needs and develop personalized care plans.
• Advocate for patients and ensure effective communication with stakeholders.
• Coordinate services across hospital, clinic and home care settings.
• Evaluate needs and determine eligibility for benefits.
• Document all casework activities accurately and timely.
• Collaborate with internal teams to optimize patient outcomes.
• Ensure compliance with regulatory and carrier requirements.
