Qualifications

• Active and unrestricted RN licensure in Michigan.
• 3+ years of clinical practical experience (diabetes, CHF, CKD, post-acute care, hospice, palliative care, cardiac, Home Health).
• 2+ years of case management, discharge planning or home health coordination experience.
• Proficiency with MS Office applications (Word, Excel, Outlook, PowerPoint).
• Ability to work in a fast-paced high-volume environment and manage time effectively.
• Working knowledge of medical terminology and digital literacy.

Responsibilities

• Coordinate and facilitate comprehensive care for individuals by assessing needs and developing personalized care plans.
• Act as liaison with members, families, providers, insurers and healthcare personnel.
• Document case work activities and monitor progress to promote positive outcomes.
• Interact with members telephonically or in person, including home visits when needed.
• Apply laws and regulations related to rehabilitation services and insurance requirements.
• Coordinate services across healthcare providers and community resources.
