Job summary

Belle, LLC is seeking a full-time remote Telephonic Nurse Case Manager to improve clinical outcomes and care coordination for members and their caregivers. The role centers on high-quality, member-centered care delivered via telephonic case management and collaboration with care teams.

Ideal candidates will hold an active RN license with Multi-State Compact licensure and have 2+ years of clinical case management experience with older adults.

Qualifications

• Active RN license with Multi-State Compact licensure.
• Two years of clinical case management experience.
• Experience working with older adults.
• Strong organizational and time management skills.
• Excellent verbal and written communication skills.
• Empathetic with strong active listening abilities.
• Preferred: Case management certification.
• Preferred: Spanish-speaking ability.
• Preferred: Experience with remote work.
• Experience coordinating timely referrals to care and community resources.

Responsibilities

• Manage, validate, and track all member cases, ensuring timely communication with appropriate healthcare and community resources
• Leverage strong people skills to relate to members, show empathy for their situational/clinical circumstances, and advocate for the member’s best interest
• Provide feedback on BT documentation
• Communicate areas for improvement clearly to support ongoing care quality and member experience
