Job summary

CVS Health Corporation is seeking an experienced Registered Nurse Case Manager to provide telephonic case management for members with complex healthcare needs, working remotely within the United States. The role involves assessing, planning, coordinating, and managing care while helping members navigate resources and improve health outcomes.

The ideal candidate has an active RN license, at least 5 years of clinical RN experience, and willingness to obtain a Compact RN license.

Qualifications

• Active, unrestricted RN license in the state of residence.
• Willingness to obtain a Compact RN license (company-paid).
• Minimum 5 years of clinical RN experience.
• Associate Degree in Nursing (ADN).
• Ability to work Monday-Friday business hours.

Responsibilities

• Conduct telephonic assessments and develop individualized care plans.
• Identify health risks, barriers, and opportunities to improve member wellness.
• Coordinate care, referrals, and transitions to appropriate clinical programs and resources.
• Apply clinical judgment to support members with complex and co‑morbid conditions.
• Collaborate with multidisciplinary teams to achieve optimal member outcomes.
• Maintain compliance with regulatory requirements and company policies.
