Job summary

CVS Health Corporation is seeking an experienced Registered Nurse Case Manager to provide remote telephonic care coordination for members with complex health needs. You will assess, plan, and manage care, helping members access resources and navigate the healthcare system.

Ideal candidates hold an active RN license, an ADN, and 5+ years of clinical RN experience, with willingness to obtain a Compact RN license. This remote role offers a 40-hour week and competitive benefits.

Qualifications

• CM experience or knowledge of utilization management is preferred.
• RN with active, unrestricted license in state of residence.
• Ability to obtain a Compact RN license (company-paid).

Responsibilities

• Conduct telephonic assessments and develop individualized care plans.
• Identify risks and opportunities to improve member wellness.
• Coordinate care, referrals, and transitions to programs and resources.
• Apply clinical judgment for members with complex conditions.
• Collaborate with multidisciplinary teams to achieve outcomes.
• Maintain compliance with regulatory requirements and policies.
