CVS Health is seeking a remote RN Case Manager to support members nationwide via telephonic case management. The role includes applying clinical criteria, coordinating care, and addressing barriers to improve outcomes. A multi-state license and hospital experience are required.

Qualifications

Active RN license valid in multiple states via compact or state-specific license.
5+ years of RN experience with hospital setting background.
Experience documenting electronically and using a keyboard.
Experience in Oncology, Transplant, Specialty Pharmacy, Pediatrics, Medical/Surgical, Behavioral Health/Substance Abuse or Maternity.

Responsibilities

Work as telephonic case manager for fully and/or self-insured clients.
Apply clinical criteria and guidelines to assess member needs and benefits.
Use holistic approach to address barriers and improve care planning.
Confer case details in multidisciplinary conferences for better outcomes.
Perform medical necessity reviews and coordinate care with team.
