Job summary

Belle, LLC is hiring a full-time, remote Telephonic Nurse Case Manager to improve clinical outcomes and care coordination for members. You will review in-home visit documentation, conduct telephonic case management, track referrals to health and community resources, assess SDOH, and develop individualized care plans with members and their care teams.

The role emphasizes compassionate, patient-centered support, with strong communication and problem-solving skills, and prior remote work experience

Qualifications

• Active RN license with multi-state licensure.
• 2 years in clinical case management.
• Experience with older adults.
• Strong organizational and communication skills.
• Empathetic with active listening.
• Willingness to work remotely.
• Preferred: bilingual Spanish-speaking.

Responsibilities

• Review documentation from in-home visits.
• Provide telephonic case management for members.
• Facilitate and track referrals to health and community resources.
• Assess Social Determinants of Health (SDOH) and support resolution of barriers to care.
• Develop and manage individualized care plans with members and care teams.
• Manage, validate, and track all member cases with timely communication of resources.
• Provide feedback on documentation and communicate areas for improvement.
