Job summary

CVS Health Corporation seeks an experienced RN Case Manager to provide remote telephonic case management for members with complex healthcare needs. You will assess, plan, coordinate, and manage care while helping members navigate resources to improve health outcomes.

The role emphasizes patient-centered care, collaboration with multidisciplinary teams, and strict regulatory compliance. A current RN license and ADN are required, with company support for Compact licensure.

Qualifications

• Active, unrestricted RN license in home state.
• Willingness to obtain Compact RN license (company-paid).
• Minimum 5 years of clinical RN experience.
• Associate Degree in Nursing (ADN).
• Ability to work Monday–Friday business hours.
• BSN preferred.
• Case Management Certification (CCM) preferred.

Responsibilities

• Conduct telephonic assessments and develop individualized care plans.
• Identify health risks, barriers, and opportunities to improve member wellness.
• Coordinate care, referrals, and transitions to appropriate clinical programs and resources.
• Apply clinical judgment to support members with complex and co‑morbid conditions.
• Collaborate with multidisciplinary teams to achieve optimal member outcomes.
• Maintain compliance with regulatory requirements and company policies.
