Job summary

GlobalHealth is seeking an experienced RN Case Manager to oversee telephonic case management and advocate for members’ health needs. You will coordinate care with providers, educate members and caregivers, and participate in SNP MOC activities while maintaining confidentiality and HIPAA standards.

The role supports a remote work environment with state-specific exclusions; Oklahoma licensure is required and multi-state licensure is a plus.

Qualifications

• Active Registered Nurse license in the state of Oklahoma required.
• Active multi-state RN license preferred.
• Previous experience in managed care/utilization management preferred.

Responsibilities

• Conduct telephonic case management for complex, high‑risk members.
• Coordinate with providers to ensure holistic outcomes.
• Communicate with diverse backgrounds on care needs and changes.
• Coordinate interdepartmentally and with the care team.
• Educate members and caregivers on conditions and self‑management.
• Engage in SNP MOC elements: complete HRA, ICP, ICT; TOC activities post-discharge.
