Job summary

Optum is seeking an experienced Registered Nurse with comprehensive case management expertise to support high-volume member interactions by phone. This remote role requires a current RN license, a compact license where applicable, and a track record in case management within hospital or direct care settings.

You'll assess health status, address gaps in treatment, educate patients, and coordinate resources while thriving in a telephonic environment. Excellent communication is essential.

Qualifications

• Current, unrestricted RN license in your state of residence.
• Compact RN license required in compact states.

Responsibilities

• Make outbound and inbound calls to assess members' health status.
• Identify gaps or barriers in treatment plans.
• Provide patient education to assist with self-management.
• Make referrals to outside sources.
• Coordinate a complete continuum of quality care through communication with members.
• Assist with resources such as Home Health Aides or Meals on Wheels.
