Job summary

CVS Health is hiring an Integrated Care Management (ICM) Care Manager to assess, plan, implement and coordinate case management activities for members in a remote-capable role with travel in Ocean County, NJ. You will work with members, providers and community organizations to address health and social determinants of health, aiming to improve overall wellness.

The role requires an active NJ RN license, 3+ years of clinical experience, and ability to travel 25-50%.

Qualifications

• Active NJ RN license required.
• Willing to travel 25-50% of time.
• Residence in Ocean County or surrounding counties, NJ.
• Bachelor's degree preferred.

Responsibilities

• Assess, plan, implement and coordinate all case management activities for members.
• Collaborate with members, providers and community organizations to address health and social needs.
• Coordinate care for dual eligible Medicare/Medicaid populations and address barriers to goals.
