CVS Health is hiring a Nurse Care Manager for a work-from-home role with 25–50% travel across Ocean County, NJ. You will assess member needs, coordinate care, and collaborate with the healthcare team to improve outcomes.

The ideal candidate has an NJ RN license, 3+ years of clinical practice, and is comfortable traveling to meet members in their area. Remote work with occasional on-site visits is available, and a comprehensive benefits package is offered.

Qualifications

• Active and unrestricted RN license in NJ
• Willing to travel 25–50% of time using own vehicle
• Must reside in Ocean or surrounding County, NJ

Responsabilités

• Assess, plan, implement and coordinate all case management activities for members
• Evaluate medical needs and facilitate overall wellness
• Collaborate with physicians, nurses, and community resources to address barriers and optimize care
• Conduct holistic assessments and determine referrals to clinical resources
• Engage members using motivational interviewing to maximize participation
