CVS Health is seeking an Integrated Care Management (ICM) Care Manager to assess, plan, implement, and coordinate care for dual eligible members. This role requires a RN license in NJ, 3+ years of clinical practice, and 25-50% travel to meet members in their area.

Qualifications

• Minimum 3+ years of clinical practice experience.
• Must have active and unrestricted RN license in the state of NJ.
• Willing and able to travel 25-50% of their time using your own vehicle to meet members face to face in their assigned area. Mileage is reimbursed per policy.
• Must reside in Ocean or surrounding County, NJ.
• Full-time position with standard hours 8-5 pm.
• Bachelor's degree preferred; Certified Case Manager is preferred.
• Bilingual preferred.

Responsibilities

• Assess, plan, implement and coordinate all case management activities for dual eligible members.
• Collaborate with members, providers, and community organizations to address health and social needs.
• Use clinical tools and data to evaluate member needs and benefits.
• Develop proactive care plans to improve short- and long-term outcomes.
• Ensure regulatory compliance and maintain documentation in CMS systems.
