CVS Health in New Jersey is seeking a Registered Nurse Case Manager to support members in Essex County and surrounding areas. This remote role requires up to 75% field travel and a Monday–Friday schedule, 8:00 AM–5:00 PM EST.

You will develop individualized care plans, coordinate medical, behavioral, and psychosocial services, and document activities in the electronic health record while mentoring new team members to ensure high-quality, cost-effective care.

Formación

• Active, unrestricted RN license in New Jersey.
• 3+ years of clinical practice experience.
• 3+ years of home health, hospice, or a similar setting.
• Residency in Essex County NJ and NJ Choice Certification requirements.

Responsabilidades

• Conduct in-home, face-to-face assessments for members enrolled in MLTSS, D-SNP/FIDE programs.
• Develop and implement individualized care plans with interdisciplinary teams.
• Coordinate medical, behavioral, and psychosocial services for members.
• Authorize services per guidelines and support safe transitions of care.
• Document all case activity promptly in the electronic health record.
• Mentor new team members once proficiency is achieved.
