Job summary

CVS Health is seeking a compassionate Registered Nurse Case Manager for Essex County, NJ, with up to 75% field travel. This remote role supports members across Essex and surrounding counties, coordinating care from 8:00 AM-5:00 PM EST, Monday–Friday.

You will develop individualized care plans, coordinate medical, behavioral, and social services, and ensure safe transitions of care. Requires active NJ RN license, 3+ years' clinical and home health experience, and NJ Choice Certification.

Qualifications

Active, unrestricted RN license in New Jersey.
Comfort conducting field-based home visits across diverse settings.
3+ years of clinical practice experience.
3+ years in home health, hospice, or similar setting.
Residency in Essex County, NJ.
Successful completion of the NJ Choice Certification, including exam, modules and mentoring.
Reliable transportation and willingness to travel up to 75% with mileage reimbursement.

Responsibilities

In-home, face-to-face assessments for MLTSS and D-SNP/FIDE members.
Evaluate needs and coordinate services such as day care and nursing facility placement.
Develop and implement individualized care plans with members, caregivers, PCPs, and teams.
Authorize services per program guidelines and support safe transitions of care.
Document all case activity accurately in the electronic health record.
Apply clinical judgment and critical thinking to manage complex care needs.
Mentor new team members once proficient.
