Job summary

Point32Health is seeking a Care Manager – Nursing Field (RN CM) to ensure timely care management across the care continuum, including transitions of care, care coordination, and chronic disease management. The RN CM collaborates with members, caregivers, and providers to empower self-management and improve wellness.

The role requires an active RN license, a bachelor’s degree in nursing, and at least five years of clinical experience.

Qualifications

• Registered Nurse with current unrestricted license in state of residence.
• National case management certification desirable
• Bachelor’s degree in nursing required
• Minimum 5 years’ relevant clinical experience

Responsibilities

• Perform telephonic member outreach and/or face-to-face encounters to facilitate program enrollment.
• Assess member health goals and barriers to wellness; identify needs.
• Develop and revise a member-specific plan of care with care interventions.
• Complete documentation per departmental policy and regulatory standards.
• Provide targeted health education and proactive strategies for condition management.
• Coordinate safe transitions to next level of care and post-discharge needs.
• Collaborate with interdisciplinary care team to improve member outcomes.
• Present high-risk members at interdisciplinary rounds; continue professional growth.
