Job summary

Point32Health in the United States is seeking an experienced Care Manager – Nursing Field (RN CM) to ensure timely care management across the continuum, including transitions of care, care coordination and navigation, complex case management and population health interventions.

The RN CM will collaborate with members, caregivers and providers to develop and implement member-specific plans of care, monitor outcomes, and empower self-management.

Qualifications

• Registered Nurse with current unrestricted license in state of residence.
• 5 years’ relevant clinical experience.
• Bachelor’s degree or relevant equivalent experience.
• National certification in Case Management desirable.
• Understand and follow state-specific Nurse Practice Acts and licensure requirements.

Responsibilities

• Perform telephonic member outreach and/or face-to-face encounters to facilitate program enrollment.
• Provide targeted health education and proactive strategies for condition management.
• Coordinate safe transitions of care and ensure post-hospital instructions and follow-up.
• Collaborate with interdisciplinary care team to improve member outcomes.
• Maintain professional growth through ongoing learning.
