Job summary

Point32Health, a leading not-for-profit health and well-being organization, seeks a seasoned Care Manager – Nursing Field (RN CM). This role ensures timely care management across the continuum, including transitions of care, care coordination, and disease management, guided by department standards.

The RN CM collaborates with members, caregivers, and providers to develop and implement personalized care plans, empowering optimal wellness and self-management per national CM standards.

Qualifications

• RN with current unrestricted license in state of residence.
• May be required to obtain other state licensure in states where Point32Health operates.
• National certification in Case Management desirable.

Responsibilities

• Perform telephonic member outreach and/or face-to-face encounters to facilitate program enrollment and assess member needs.
• Provide targeted health education and proactive strategies for condition management.
• Coordinate safe transitions of care and follow-up to prevent readmissions.
• Collaborate with interdisciplinary care teams to improve member outcomes and participate in rounds when appropriate.
• Maintain professional growth and development through ongoing learning.
