Qualifications

• Registered Nurse with current unrestricted license in state of residence.
• National certification in Case Management desirable
• Bachelor’s degree in nursing preferred

Responsibilities

• Perform telephonic member outreach and/or face-to-face encounters to facilitate program enrollment.
• Develop member-specific plan of care with providers and caregiver involvement.
• Document in applicable platform per policy and regulatory standards.
• Provide health education and proactive strategies for condition management.
• Coordinate safe transitions to next level of care and prevent readmission.
• Collaborate with interdisciplinary care team to improve member outcomes.
