Job summary

Point32Health is seeking a Nurse Care Manager (RN CM) to ensure timely care management across the care continuum, including transitions of care, care coordination, and disease management. The role emphasizes empowering members to improve wellness, collaborating with families and providers, and applying national CM standards.

The ideal candidate holds an RN with an unrestricted license, 5+ years of clinical experience, and a Bachelor’s degree (or equivalent).

Qualifications

• Registered Nurse with current unrestricted license in state of residence
• Minimum 5 years of relevant clinical experience
• Bachelor's degree or equivalent experience required
• National case management certification desirable

Responsibilities

• Perform telephonic member outreach and/or face-to-face encounters to facilitate program enrollment.
• Assess members holistically to identify needs, health goals, and barriers to wellness.
• Develop and revise member-specific plans of care with care manager interventions.
• Document activities in the designated platform per policy and regulatory standards.
• Provide targeted health education and proactive strategies for condition management.
• Coordinate safe transitions of care and follow-up services to prevent readmission.
• Collaborate with interdisciplinary care team to improve member outcomes.
• Attend interdisciplinary rounds for high-risk members when appropriate.
• Pursue ongoing professional development through self-directed learning.
