Job summary

Point32Health is seeking a Care Manager – Nursing Field (RN CM) to ensure timely care management across the continuum, including transitions of care, care coordination and disease management per department guidelines.

The RN CM collaborates with members, caregivers, and providers to develop and implement a member-specific plan of care, empowering wellness and self-management, in a national care delivery environment.

Qualifications

• RN licensure and current state license
• Bachelor’s degree in nursing required or equivalent experience
• National Case Management certification desirable

Responsibilities

• Perform telephonic outreach and coordinate program enrollment.
• Provide health education and condition management strategies.
• Coordinate safe transitions of care and post-discharge follow-up.
• Collaborate with interdisciplinary care teams to optimize member outcomes.
• Maintain professional growth through ongoing learning
