Job summary

Humana Inc. is seeking a Care Manager, Telephonic Behavioral Health 2, to assess and address members’ behavioral health needs in a telephonic setting.

The role emphasizes guiding members and families to appropriate resources and coordinating care to improve wellness in a remote environment. The incumbent will use evidence-based approaches, monitor progress, and adjust plans as needed, while following Humana policies and HIPAA requirements.

Qualifications

• Must hold a current valid social service license (LMSW/LCSW/LPC/LMHC).
• Master's degree in a behavioral-health field from an accredited university.
• Minimum of 2+ years experience as a behavioral health professional.
• Experience with behavioral change, health promotion, and coaching.
• Proficiency with Microsoft Office programs and related systems.

Responsibilities

• Assess and evaluate members' needs to achieve and maintain optimal wellness.
• Guide members and families toward resources and care coordination.
• Develop and monitor member care plans and track progress toward outcomes.
