Qualifications

• 2+ years of behavioral health experience
• Experience with behavioral change and coaching
• Proficiency with Microsoft Office programs

Responsibilities

• Assess and evaluate members' needs to guide toward resources and support.
• Identify and resolve barriers hindering effective care.
• Monitor progress toward desired outcomes and adjust care plans as needed.
• May create and manage member care plans in telephonic care.
• Follow established guidelines and work with minimal supervision.
