Job summary

Humana Inc. is seeking an Oklahoma-based Care Manager, Telephonic Nurse II, to assess enrollees’ health needs and coordinate resources for optimal wellness.

This remote role involves collaboration with an interdisciplinary team and may require in-person visits or travel to Humana Oklahoma offices for meetings and training. The incumbent will perform telephonic and face-to-face assessments, create care plans, and guide families toward appropriate supports, with a focus on physical health

Qualifications

• Active clinical experience with RN/LPC/LCSW credentials
• 2+ years of care management experience
• Experience coordinating Medicaid/Medicare services
• Proficient in Microsoft Office applications (Word/Excel/PowerPoint)

Responsibilities

• Assess and evaluate enrollees to achieve or maintain optimal wellness.
• Guide enrollees/families toward resources and coordinate care.
• Develop enrollee care plans aligned with health goals and outcomes.
• Perform telephonic and face-to-face assessments and evaluations as needed.
• Collaborate with the interdisciplinary care management team and providers.
• Ensure ongoing monitoring of care, adjust plans to improve outcomes.
• Possibly travel to Humana Oklahoma office for meetings and training.
