Job summary

Humana is seeking a Care Manager, Telephonic Nurse to assess enrollees' needs and guide families toward appropriate resources. You will assess, plan, and monitor care to optimize wellness, using telephonic and face-to-face interactions as needed.

Required are an active RN license or LPC/LCSW, with 1 year clinical experience and 2+ years in care management, plus experience with Medicaid/Medicare enrollees. This is a remote role based in Oklahoma.

Qualifications

• Active RN license, LPC, or LCSW required.

Responsibilities

• Assess enrollees' needs and requirements.
• Guide enrollees and families towards appropriate resources.
• Employ strategies to manage behavioural, physical, environmental, and psycho-social health needs.
• Monitor assessments to ensure progress toward outcomes.
• Create enrollee care plans.
• Perform telephonic and face-to-face assessments.
• Collaborate with providers and community services for quality outcomes.
• Follow established guidelines and procedures.
• Engage with the interdisciplinary care management team.
• Coordinate in-person meetings between care managers and enrollees.
